In Bulgaria, we generally have only three names: a given name, a patronymic and a family name. This is a relatively recent system, I believe. It must have taken form after the Liberation in 1878, so late 19th - early 20th century.

In Spain I think there's nothing that would technically be a middlename, although some people that are not from here may mistake our two surnames for baing a middle + a last name, which is not the case.

Now it is obsolete but a few decades ago even more than two names.
